AMIA and HL7 are partnering again to bring an exciting application competition to the AMIA 2023 Annual Symposium. As patients move around the healthcare ecosystem, their electronic health records must be available, discoverable, and understandable. The competition is intended to highlight the best and most innovative FHIR® implementations.

Dave Carlson will be presenting the MyCarePlanner application, developed as part of a joint project between the Agency for Healthcare Research and Quality (AHRQ) and the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK).

MyCarePlanner is a SMART on FHIR application that aggregates and summarizes prioritized data across multiple healthcare sites to support comprehensive, person-centered shared care planning for people with multiple chronic conditions.
